Do assessments to determine patients’ psychosocial needs. Develop care planning with healthcare teams. Provide counseling support to patients and families dealing with illness and hospitalization. Help with discharge and post-discharge care.
Connect each patient with community resources and support services. 2 or more years of experience in medical or healthcare social work is needed. Basic understanding of the psychosocial aspects of illness and hospitalization. Communication and interpersonal skills required and ability to work together with a multidisciplinary team.
